Telephone appointments available for existing patients and charged the same fees as office visits.
Email communication is not typically offered and is discouraged at this time due to the unreliability of received communication. If pre-authorized by Dr. Miller, there is a minimum response fee of $50. If more time is necessary to respond, a telephone or office visit will be recommended.
Insurance Carriers
It is the patient’s responsibility to check their insurance coverage to determine if their specific plan covers services provided by a licensed Naturopathic Doctor. Dr. Miller is not a preferred provider on any plans and the office does not directly bill any insurance companies. A coded bill is available upon request for the patient to submit to their insurance carrier for reimbursement.

About Your First Office Visit
Prior to your first appointment, you will need to download your new patient intake forms, complete them, and mail them to the address below. The questionnaires are used to help identify areas where you may have health problems or risks. In addition, Dr. Miller will gain a comprehensive picture of your health status and will be able to focus on the health issues that concern you most.
Your first office appointment is 1.5 hours and usually consists of a detailed interview, a general physical exam, and initial recommendations for an individualized care plan. If your particular case is more complex, the doctor may recommend deferring the physical exam to a later appointment to allow time for a more comprehensive interview.
